I Need Your Help: How Do People Change?

About a year ago, I conducted an informal survey via Facebook on the following question: Why DON'T people change despite knowing that it would be in their best interest? I appreciated all of the great responses!

At the present moment, I would like to pose a similar, yet much different, question: How DO people change knowing that it is in their best interest? In other words, how do they work through fear, resistance, apathy, rationalizations, ambivalence, laziness, etc.?
